Welcome to Personality Matters! Coaching and counselling is a collaborative process designed to help you explore issues, come to a greater understanding of yourself, overcome roadblocks, and achieve your aspirations. It involves a relationship between you and a trained professional who has the competency and willingness to help you journey towards your goals in a safe and non-coercive setting. We believe each person has the potential to live a fulfilling life, and they can be architects of their own transformation.
Before you begin your journey of self and professional development with us, please take a few moments to read through this document, which contains important information about our services and policies. At the end of this page, you are required to fill up your personal particulars and click the “submit” button to indicate your consent.
By submitting this form, you agree to the following terms:
Participation: You hereby acknowledge that you are participating in our programs and services voluntarily and without duress.
Confidentiality agreement: The information obtained from you will remain confidential and will not be shared with anyone outside Personality Matters except under the following conditions: (1) At your request; (2) Perceived risks to commit serious harm to yourself or others; (3) Information indicating the abuse of children; and (4) Valuable information during a medical emergency.
Appointments and fees: Sessions are charged at SGD 240 per hour (online and at our office in Singapore), SGD 350 per hour (off-site and outdoors in Singapore), and SGD 600 per hour (outside of Singapore). We appreciate prompt arrival for appointments. Please notify us beforehand if you will be late. A full fee may charged for missed appointments without advanced notice (of at least 24 hours) or valid reasons. If sessions exceed the allotted 60 minutes, charges for any additional time will be pro-rated in 30-minute blocks. The fee level is subject to change if conditions make it necessary. But we will inform you of this in advance.
Payment: You will receive an invoice at the start of each month for all sessions conducted within the previous month. We currently accept payment transfer via internet banking – details will be provided in the invoice.
Release of Liability: You hereby release and forever discharge and hold harmless Personality Matters, its staff, volunteers, representatives and agents from any and all liability, claims, and demands in respect to any loss, damages, accidents, personal injury, illness, death, or any other irregularities beyond the control of Personality Matters that may arise during or as a result of your participation in our programs and services, whether caused by negligence or otherwise. You also agree that your assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against or sue Personality Matters in connection with any of the matters covered by this informed consent and release form.
