Consent for Personality & Behavioural Profiling (with parental consent)

Before you begin your journey of personal and professional development with us, please take a few moments to read through this document, which contains important information about our services and policies. By submitting this form, you agree to the following terms:

Participation: You hereby acknowledge that you are participating in our programs and services voluntarily and without duress.

Confidentiality agreement: The information obtained from you will remain confidential and will not be shared with anyone outside Personality Matters except under the following conditions: (1) At your request; (2) Perceived risks to commit serious harm to yourself or others; (3) Information indicating the abuse of children; and (4) Valuable information during a medical emergency.

Appointments: The time and location of the consultation session will be determined based on mutual agreement. Any additional consultation or coaching session(s) will be charged separately.

Release of Liability: You hereby release and forever discharge and hold harmless Personality Matters, its staff, volunteers, representatives and agents from any and all liability, claims, and demands in respect to any loss, damages, accidents, personal injury, illness, death, or any other irregularities beyond the control of Personality Matters that may arise during or as a result of your participation in our programs and services, whether caused by negligence or otherwise. You also agree that your assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against or sue Personality Matters in connection with any of the matters covered by this informed consent and release form.
